Bariatric surgery after GLP-1: Mounjaro not working, or the weight came back?

Two groups write to us from the UK every week: people who say ‘Mounjaro not working’ after months on a full dose, and people who lost weight, stopped, and watched it return. This page is for both. It explains when bariatric surgery after GLP-1 makes sense, which operations exist, what the gastric balloon can do at a lower BMI, and what it costs in İzmir.

Op. Dr. Volkan Arayıcı, bariatric surgeon in İzmir: Mounjaro not working or weight regained – surgery options

Which one are you?

Mounjaro not working, or weight gain after stopping Mounjaro: two different problems, one next step

Bariatric surgery after GLP-1 is not one story but two. Some people never respond to the injections; others respond well and regain everything when they stop. The medicine is the same, the biology is different, and both groups end up asking the same question: what now?

Group 1: the injections are not working

GLP-1 non-responder

You have reached a full dose, taken it for months, and the scales barely move. Roughly one in ten people is a GLP-1 non-responder: less than five per cent lost after six months. The online pharmacy will suggest another dose step. Sometimes that is right; often it only delays the real decision.

Group 2: it worked, you stopped, it came back

Weight regain after the injections

The most common message we receive. Weight gain after stopping Mounjaro, Wegovy or Ozempic is expected, not exceptional: in the trials about two thirds of the lost weight returned within a year. Nobody told you that the effect ends with the last injection.

What both groups share: you already know how your body reacts to reduced hunger, and you know that a treatment you cannot sustain for life is not a solution. That knowledge is the starting point for bariatric surgery after GLP-1, and it makes the consultation shorter, not longer.

Group 1

Mounjaro not working: why some people do not respond, and when to stop trying

Searches for "Mounjaro not working", "Ozempic not working" and "why am I not losing weight on Mounjaro" have exploded in the UK, and most of the answers come from pharmacies that also sell the pen. Their advice is always the same: wait, increase the dose, eat more protein. Some of it is correct. What is missing is the honest end point.

  1. 1

You are still on a starting dose

2.5 mg and 5 mg are run-in doses meant to reduce nausea, not to lose weight. Judging Mounjaro not working after three weeks is too early; judge it at a treatment dose.

  2. 2

    You are a genuine non-responder

Between ten and fifteen per cent of people lose less than five per cent of their body weight even on the highest dose. NICE's rule for semaglutide is to stop if less than five per cent has been lost after six months; the same benchmark is used for tirzepatide. A GLP-1 non-responder does not become a responder by injecting longer, and Ozempic not working after six months at 2 mg means the same thing as Mounjaro not working at 15 mg.

  3. 3

    The hunger is still there

‘Mounjaro not working, still hungry’ is one of the most searched phrases. If appetite barely changes at 10 or 15 mg, the drug is not reaching the switch it is meant to flip. That is exactly what a gastric sleeve does anatomically: it removes the part of the stomach that produces most of the hunger hormone ghrelin.

  4. 4

Side effects stop you reaching the dose

Nausea, vomiting, constipation, hair loss or noticeable muscle loss force many people to stay on a low dose, where the effect is small. A treatment you cannot tolerate at its working dose is, for you, a treatment that does not work.

Our rule of thumb: six months at the highest dose you tolerate, less than five per cent lost, and a BMI still over 35, or over 30 with type 2 diabetes, sleep apnoea or joint disease. At that point bariatric surgery after GLP-1 is not giving up; it is choosing the tool that works independently of the hunger hormone. What surgery does for diabetes specifically is explained under gastric sleeve for diabetes.

Group 2

Weight gain after stopping Mounjaro or Wegovy: what happens, and what to do about it

Semaglutide and tirzepatide slow gastric emptying and dampen the hunger signal only while the drug is in your blood. Stop, and within weeks the old appetite is back, usually with the old weight following. In SURMOUNT-4, people who switched from tirzepatide to placebo regained about two thirds of their loss in a year; the STEP-1 extension showed the same for semaglutide. Weight gain after stopping Mounjaro is the drug wearing off, not a personal failure.

The practical consequence is that the injection is a lifelong therapy. Privately in the UK that means £150 to £250 a month, roughly £1,800 to £3,000 a year, with no end date. Some people accept that. Many of those who write to us do not, either because of the cost, the side effects or simply because they do not want to inject for the rest of their life.

Three things help before you decide anything: note your lowest weight on the injections and your current weight; write down what changed in your eating when you stopped; and check whether any conditions returned, such as blood pressure or blood sugar. Those three numbers tell us most of what we need for a first assessment. Why the scales climb again after a sleeve, and how that differs, is covered under weight regain after gastric sleeve.

Your options

The surgical options after GLP-1: sleeve, bypass or balloon

There is no special operation for people who did not respond to the injections or regained after stopping. The options are the same four procedures we offer everyone, and the choice is made individually from your BMI, reflux, diabetes, any previous abdominal surgery and what you want from the result. Dr. Arayıcı makes that decision with you, not from a table; the table only shows where each option usually fits.

Details of each procedure: gastric sleeve in Turkey, mini gastric bypass in Turkey, gastric bypass in Turkey and gastric balloon in Turkey. If you have had bariatric surgery before and regained despite it, the routes are on gastric revision surgery in Turkey.

Where the balloon fits after the injections: many people who regained after Mounjaro are now at a BMI of 30 to 35, too low for surgery without a related condition, but unhappy with a lifelong pen. A six-month balloon gives the same early fullness the injection gave, without medication, and the six months are used to build habits that outlast it. It is honest to say that, like the injection, the balloon is not permanent.

The comparison

Mounjaro vs gastric sleeve: what really separates them

Mounjaro vs gastric sleeve cannot be answered with better or worse, only with mechanism, duration and cost. The injection produces pharmacologically what the operation produces anatomically: less hunger, earlier fullness. The difference lies in what remains when you stop.

Realistic figures: people lose 10 to 15 per cent of their body weight in the first year on semaglutide and up to 20 per cent on tirzepatide, while they inject. After a gastric sleeve or bypass it is 25 to 35 per cent within one to two years, and there is no stopping. At a starting weight of 20 stone that is the difference between about 2 stone and 6 stone.

On cost the sums flip after a year: £1,800 to £3,000 a year for the pen, indefinitely, against £3,000 once for a sleeve or £3,500 for a bypass. All prices are on the page gastric sleeve surgery price. Mounjaro vs gastric sleeve is therefore rarely a question of which works; it is a question of how long you want it to work.

Before the operation

Gastric sleeve after Ozempic or Mounjaro: is the operation any different?

Surgically, no. A gastric sleeve after Ozempic, Wegovy or Mounjaro is performed exactly like one without prior medication: the same sleeve, the same oversewn staple line, the same four nights. If you lost some weight on the injections, you arrive with a smaller liver and less abdominal fat, which shortens the operation and lowers the anaesthetic risk. If you lost nothing, you are simply where every other patient starts.

Stopping the injection is not a requirement. We recommend taking your last injection about a week before the operation, because GLP-1 delays gastric emptying and the stomach should be empty under anaesthesia. You do not need a liver-shrinking diet, and you do not need to stop weeks in advance or taper. If your last dose was three days before you travel, tell us and the anaesthetic team plans around it. How we handle safety in general is under how safe is gastric sleeve surgery in Turkey.

How it works

From injections to surgery: bariatric surgery after GLP-1 in four steps

  1. 1

Decide whether the injections have had their chance

Six months at a full dose with under five per cent lost, or regain after stopping with a BMI over 35, or over 30 with a condition: time to talk about surgery. Between 30 and 35 without a condition: the balloon is the option to discuss.

  2. 2

    First assessment on WhatsApp

Height, weight, lowest weight on the injections, which product, how long, and any other conditions. Op. Dr. Volkan Arayıcı replies personally, usually the same day, and tells you honestly whether surgery, a balloon or staying on the medication makes sense.

  3. 3

Last injection about a week before surgery

Not a requirement, but our recommendation. No liver-shrinking diet, no waiting list.

  4. 4

Surgery on day two, four nights in hospital

Day one: arrival, a blood test, and you sleep on the ward. Day two: ultrasound, echocardiogram, cardiology, chest and anaesthetic checks in the morning, surgery around midday. Four nights in hospital, no hotel, day five transfer to the airport.

The operation itself takes 45 to 75 minutes, laparoscopically through small incisions. The staple line is oversewn and the seal is tested during the operation; we have not used routine drains for more than five years.

Cost & the NHS

What bariatric surgery after GLP-1 costs, and why the NHS is rarely the route

Gastric sleeve from £3,000, mini gastric bypass and gastric bypass from £3,500, gastric balloon from £1,200, all-inclusive. The surgery packages cover pre-operative tests, the operation, four nights in hospital, a companion staying in your room with meals, an interpreter, transfers, medication during your stay and twelve months of follow-up by video call and WhatsApp. Flights are not included; the package begins when you land in İzmir. Payment is on the day of surgery, without deposit and without instalments.

On the NHS, Wegovy and Mounjaro are prescribed only through specialist tier 3 weight-management services with strict criteria and long waits, which is why most people in the UK pay privately. NHS bariatric surgery runs through the same pathway, typically over one to two years, and surgery abroad is not funded. What UK patients say about their week in İzmir is collected under gastric sleeve Turkey experiences.

Why we do not sell injections: we are surgeons, not a pharmacy. If you want the injection, get it from a doctor. If you want to know whether surgery or a balloon makes sense for you, you get an honest answer from us, including "not yet".

Frequently asked questions

Bariatric surgery after GLP-1: your questions answered

What counts as a GLP-1 non-responder?

In practice: less than five per cent of your starting weight lost after about six months on the highest dose you tolerate. That is the benchmark NICE uses for stopping semaglutide, and the same yardstick is applied to tirzepatide. A GLP-1 non-responder is not doing anything wrong; the hunger signal simply does not react to the drug in everyone.

Is Ozempic not working the same problem as Mounjaro not working?

Yes. Ozempic and Wegovy contain semaglutide, Mounjaro contains tirzepatide; both act on the same hunger pathway. If Ozempic is not working after six months at the highest dose you tolerate, switching to Mounjaro helps some people, but a true non-responder to one often responds weakly to the other. That is when bariatric surgery after GLP-1 belongs on the table.

Mounjaro not working after three months: should I wait?

Yes, if you are still on a starting dose. The first weeks at 2.5 mg or 5 mg are a run-in, not the treatment dose. If you have reached 10 mg or more, taken it for six months and lost under five per cent, more months rarely change the outcome. That is the point to look at bariatric surgery after GLP-1.

Is weight gain after stopping Mounjaro or Wegovy normal?

Yes. In the SURMOUNT-4 trial people who switched from tirzepatide to placebo regained about two thirds of what they had lost within a year; the STEP-1 extension shows the same for semaglutide. Appetite and gastric emptying return to their old pattern because the drug is no longer there. It is pharmacology, not a lack of discipline.

Is there a special operation for people who did not respond to GLP-1?

No. Whether you did not respond to the injections or regained after stopping, the operations are the same as for anyone else: gastric sleeve, mini gastric bypass, gastric bypass or, at a lower BMI, a gastric balloon. Which one suits you is decided individually from your BMI, reflux, diabetes and any previous surgery, not from how the injections went.

Mounjaro vs gastric sleeve: which is better?

Neither is better in the abstract. Mounjaro works while you inject and costs £150 to £250 a month; a gastric sleeve changes the stomach once and the effect stays. With a BMI under 30 the injection is usually the right tool; with a BMI over 35, or over 30 with diabetes, surgery gives the more durable result.

Can I have a gastric balloon after stopping Mounjaro?

Yes. A six-month gastric balloon costs £1,200 with us and suits people with a BMI between 30 and 35 who regained after the injections but do not want, or do not yet qualify for, surgery. It is placed and removed endoscopically without an operation. Like the injection, its effect is temporary; the six months are meant for building new habits.

Is a gastric sleeve after Ozempic or Mounjaro riskier?

No. A gastric sleeve after Ozempic is the same operation; if you lost some weight on the injections you come into theatre with a smaller liver and a lower anaesthetic risk. Our only recommendation is to take your last injection about a week before surgery. You do not need to stop weeks in advance.

What does bariatric surgery after GLP-1 cost in Turkey?

Gastric sleeve from £3,000, mini gastric bypass and gastric bypass from £3,500, gastric balloon from £1,200, all-inclusive except flights. Surgery packages include tests, the operation, four nights in hospital, a companion in your room, an interpreter, transfers and twelve months of follow-up. Payment is on the day of surgery, no deposit, no instalments.

How long do I stay in İzmir?

Five days for surgery. On the day you arrive we only take a blood test and you stay on the ward. The next morning come the ultrasound, heart and anaesthetic checks, and the operation follows around midday. Four nights in hospital in total, no hotel, and a transfer to the airport on day five.

Can I take Mounjaro or Ozempic after a gastric sleeve?

Yes, under medical supervision, and for weight regain years after surgery it is often the first step before any revision is considered. Nausea and fullness can be stronger because the stomach is already small. Whether a second operation is needed is decided by an endoscopy, not by the scales alone.
